
ISDE Recommendation for Safe GI Endoscopy During the Mpox Outbreak
The article titled “Best Practice of Gastrointestinal Endoscopy during Mpox Upsurge: An Indonesian Society for Digestive Endoscopy Recommendation” has been officially published in Clinical Endoscopy on July 30, 2025. This comprehensive guideline was developed by a multidisciplinary team of experts in gastrointestinal endoscopy across Indonesia, including Dr. Rabbinu Rangga Pribadi, SpPD, K-GEH; Dr. Ahmad Fariz Malvi Zamzam Zein, SpPD, FACP; Dr. Achmad Fauzi, SpPD, K-GEH; Prof. Dr. Abdul Aziz Rani, SpPD, K-GEH; Prof. Dr. Marcellus Simadibrata, PhD, SpPD, K-GEH, FACG, FASGE; and many other respected physicians from the Indonesian Society for Digestive Endoscopy (ISDE).
This recommendation was driven by the global rise of Mpox cases, which has raised significant concern regarding the potential transmission of the virus during gastrointestinal procedures. Although no direct cases of Mpox transmission via endoscopy have been reported, the possibility remains through respiratory droplets, mucosal contact, and contaminated instruments or surfaces acting as fomites.
The risk of nosocomial transmission is particularly concerning, given the close physical contact involved in endoscopic procedures and the repeated use of medical equipment. These concerns are valid not only in endemic regions but also in non-endemic settings, underscoring the urgent need for strengthened infection prevention protocols.
This publication provides practical guidance to ensure endoscopic procedures remain safe and effective amid the Mpox outbreak. Key recommendations include thorough pre-procedural screening, proper use of personal protective equipment (PPE), meticulous disinfection after procedures, and the vaccination of high-risk healthcare workers. With these measures in place, gastrointestinal endoscopy can continue to be safely performed while minimizing transmission risk.
This article serves as a vital reference for healthcare professionals, hospital administrators, and policymakers in enhancing preparedness and response to Mpox in clinical settings.
